About Service Level Assignments and Home Visit Tracking

Service level and FSW assignments are tracked only through the Intake Form and the History Form, not from the Monthly Contact Log or Home Visit Log.  The Monthly Case Status Summary also lists participants who have enrolled but have no service level assigned for a given month.

Overview of Service Level Tracking Reports

Note that in all service level tracking reports described below, a participant is tracked from the time they are first assigned a service level until termination.  It is important to understand that this is not the same service period used to calculate retention.  Retention is calculated as each participant’s period of active service (i.e. the time period between the defined Service Start Date and Service End Date, most typically between the first home visit date and last home visit date).

Service begins at the date service level assigned.  If this date is not recorded, date accepted HFA services is used as a proxy.

Service ends at the last day for which PIMS knows the participant was provided services:

  • If the termination date occurs before the cutoff date, the report will display the termination date.
  • If the latest Info Change date occurs after the cutoff date, the report will display the date of the latest info change record.
  • Otherwise, the report will display the cutoff date.

Monthly Contact Log

  • Monthly Contact Log by Month
  • Monthly Contact Log by Participant

These reports strictly present data entered in the Monthly Contact Log.  However, service level and FSW assignments are determined from the Intake Form and the Info Change Form.  If a participant is not assigned a service level or FSW at a given month, these fields will appear blank.

Home Visit Log

In Home Visit Activities report, Total Months in Program is calculated consistently with other retention calculations- i.e., from the Service Start date (enrollment date) to the Service End date as defined by the program.

  • Monthly Home Visit Management

Service begins at the date service level assigned.

Service ends at the termination date or assigned cutoff date, whichever occurs first.

Case is considered active regardless of whether there is a Monthly Contact Log for the given month.

Monthly Home Visit Management report counts home visits from Monthly Contact Log or Home Visit Log, as specified in site definitions.

Home Visit Tracking

  • Home Visit Completion Summary
  • Monthly Case Status Summary

Service begins at the date service level assigned.

Service ends at the termination date or assigned cutoff date, whichever occurs first.

Case is considered active regardless of whether there is a Monthly Contact Log for the given month.

If a participant switches their FSW or service level during a month, the Home Visit Completion Summary will track the FSW or service level used for the greatest portion of the month.
