Instrument Information Form

The Instrument Information form allows you to enter evaluation tools used by your site to assess participant progress and child development.  To add an instrument, click the Add New button.  To edit the information for an instrument, double click on its name from the list.  Is it necessary to define the items in the drop-down menus?

The data tracking form field is used to link an instrument definition to a specific outcome tracking form.  The following outcome tracking forms are currently built into PIMS:

  • HFPI Form
  • Edinburgh Form
  • ASQ Form
  • ASQ-SE Form

If an outcome tool that you use is not tracked in PIMS, you will need to use your own tracking system (such as an Excel spreadsheet) to track scores this tool.  For any such tool, you will still need to specify whether it is intended for child development screens of family screens.

  • Generic Child Development Screen
  • Generic Instrument Admin

Instrument Schedule Form

For each instrument that your site uses, you can specify the administration schedule and checkpoints.  From the site definitions, go to Instruments and select an instrument from the list to update.

In the example below, the site has selected all the administration intervals available for Ages and Stages Questionnaire from two months to three years.

From the Instrument Schedule Form, you can then select Checkpoints.  The Checkpoints feature allows you to specify the minimum number of times an instrument needs to be administered for a participant or child within a specific time frame.  In the example below, the site specifies that at least one Ages and Stages needs to be administered every six months for each target child.
